I understand that Luminaries Inc. (“Luminaries”) offers various wellness programs (“Program”) involving meditation and physical movements as well as an opportunity for relaxation, stress re-education and relief of muscular tension (collectively the “Program Activities”) and that my participation in Program Activities is voluntary.

 

I understand that my participation in Program Activities may include activities that may be hazardous to me. As is the case with any physical activity, the risk of serious or disabling injury or even death is always present and cannot be entirely eliminated.

I acknowledge and agree that that the Program Activities are not a substitute for medical attention, examination, diagnosis or treatment.

I further understand and agree that my participation in Program Activities is not recommended and is not safe under certain medical conditions. To the extent I have a medical condition, I have consulted with a licensed medical doctor, who has authorized my participation in Program Activities. I affirm that I am physically fit to participate in Program Activities. I realize that I am participating in the Program Activities classes at my own risk.

 

As a condition of my participation in the Program, I hereby expressly assume risk of injury, harm or death from the Program Activities. I further agree to release and hold harmless Luminaries and its successors and assigns from any and all liability, claims, and demands of whatever kind and nature, either in law or in equity, which may arise from or in any way connected with my participation in Program Activities. I understand and agree that I will not now or at any time in the future bring any legal action against Luminiaries, and that this release, hold harmless and waiver of liability agreement is binding on me, my heirs, my spouse, my children, my legal representatives, my successors and my assigns.
